1) How are you feeling? 2) Something positive that has happened since our last meeting? 3) Something you are looking forward to? 4) What would you like to gain/find out during the session?
Okey early for you Danyaal so let’s check in and try to get yours done 🙂
Amy you have had your first NQP in a while so the check in is a good way for us to gauge how everyone is but also focus what we want to gather from the session
I can guess your 2) Amy and was going to announce but will let you share 🙂Amy – 19:12
Ha Lisa, yes definitely!! Thanks!Danyaal – 19:12
1)Feeling okayish, feel quite busy/stressed with wedding prep, ramadhan approaching and staying on top of my caseload! But Im managing! 2) Very good feedback on my sessions from choices, and booked a honeymoon finally! 3) Im taking over from amys client in a few weeks, looking forward to that ! 4) Some more about stuttering session plans !Lisa Franklin – 19:13
oh honeymoon – where di you decide off topic but important he heVictoria – 19:13
1) I’m feeling very tired at the moment with work and finding it difficult to maintain a happy work/life balance 2) I’m enjoying working in a school with secondary aged kids 3) looking forward to ten days in tenerife in july! 4) I’d like advice on therapy with a 4 year old, still struggling with this one!Danyaal – 19:13
Malaysia, borneo !Rosie – 19:14
1) Tired! 2) My school setting achieved ‘outstanding’ in the mock ofsted, with special emphasis on the SLT department 🙂 3) I’m looking forward to a new client I’m getting handed over 4) Opinions on one of my clients 🙂
oooo how exciting, Danyaal!Lisa Franklin – 19:15
Well if everyone else can hold fire for 5 minutes
We can ensure Danyaal gets his 4)
Firstly, work/life balance is difficult particularly with big events. I have found scheduling in time (and sticking to it!) just for your own personal relaxation works wonders. You also apporach things with a clearer mindAmy – 19:16
1) Feeling a bit stressed and exhausted with currently having 3 jobs and struggling to keep on top of things! but that will be changing soon as… 2) I’ve finally got my first band 5 NHS SLT job woohoo! 😀 3) Looking forward to handing clients over to Danyaal and possibly Rachel C 4) Some advice on discharging a client who doesn’t feel ready but I think is!Lisa Franklin – 19:16
Also don’t drink diet drinks ha ha
I have read some new research which shows caffiene and aspartame mixed cloud your judgement LOLRachel Harrison – 19:17
What age are you working with Danyaal?Lisa Franklin – 19:17
Now then Danyaal – Dysfluency session plansRachel Harrison – 19:17
Congrats Amy : )Danyaal – 19:17
adultsLisa Franklin – 19:17
age? type of stammer? stage?Rosie – 19:18
congrats amy 🙂Danyaal – 19:18
This one is 28, 37, covert stammerers
Just had initial ax
one i did, one amy didLisa Franklin – 19:19
Was this the clutter Amy?Amy – 19:20
No that one is hopefully going to Rachel C, this is the very covert one when I didn’t see any evidence of the stammer in the ax and first sessionRachel Coulter – 19:21
Yes Amy, will have to have a catch up about this 🙂Lisa Franklin – 19:22
okay okay
Well covert I always start with identification
Danyaal have you seen the dysfluency handbook?Amy – 19:22
yea def Rachel, I will send you through some more info hopefully on wed evening or thurs morningDanyaal – 19:23
ee
likewise, my 28 year old is also covert stammerer but I did see the evidence of his stammer. He claims to have had his stammer for 5 years , but I Quried it. No, where is it available?Lisa Franklin – 19:23
Okay, It is used frequently by Stav and I – I will get this scanned asap but we may need another one considering the number of requests for it
So I will order oneDanyaal – 19:24
ok awesomeLisa Franklin – 19:24
I have a girl reported from childhood and very minimal but it is because her avoidance is massiveDanyaal – 19:24
I love handbooks!Amy – 19:24
I have a few sections of the handout scanned too so will share those
*handbookDanyaal – 19:24
Brilliant thanks amy
On that guys, I have to dash 🙁Lisa Franklin – 19:25
okay Danyaal email me

bye!Danyaal left the conversation – 19:26
Rachel Coulter – 19:27
1) I’m feeling pretty tired also but good. Enjoying working with a varied client base and facing new challenges.Lisa Franklin – 19:27
🙂Rachel Coulter – 19:27
2) Something positive :- Just found out today that I got onto the SLTA waiting list for the Northern Trust 🙂Lisa Franklin – 19:28
wooooh fantastic news 🙂Amy – 19:28
yay well done!Rachel Coulter – 19:28
3) I am going to a nice black tie event at the end of the month which I am quite looking forward to
Thanks guys 🙂Lisa Franklin – 19:29
no doubt with amazing shoesRachel Coulter – 19:29
4) Em I know there isn’t very much time but probably a bit of information or advice on how to write a report following assessment. I am not sure of the layout of a report as I have never written one independently before
Haha yes Lisa though have to keep the heels a bit lower these days due to height issues lolLisa Franklin – 19:32
OKay so on the report writing front the SM will be different to standard reportsRachel Coulter – 19:32
ok…Lisa Franklin – 19:33
However, as she may not have SM I would outline findings in the key headings, exphasis on areas for – Play, Social Skills, Expressive Language
Receptive Language and attentionRachel Coulter – 19:34
ok great. Do you have a sample report you could send me that I could get the general idea from?
Oh and how long should it be?Lisa Franklin – 19:34
Yes of course
Reports are around 2-3 pages. 4 if detailed
Usually take 1 hour
but if your first one it may take considerably longerRachel Coulter – 19:35
Ok thanks 🙂 I imagine it will get easier once i’ve done one or two, just because it’s the first one i’m a bit lost lolLisa Franklin – 19:37
ha!
It gets much easier
Rachel harrison will advocate for that
shes getting very good at writing reportsRachel Harrison – 19:38
Lol, Yes, I’m still a bit slow at writing but I think improving!?Rachel Coulter – 19:38
Glad to hear that Rachel! I’m slightly freaking out lol in case I do it wrong ( You know what i’m like Lisa)Lisa Franklin – 19:38
absolutely brilliant improvements
as you become more concise its easier to watch
write*Rachel Harrison – 19:39
Thanks Lisa : )Lisa Franklin – 19:40
Okay discharging a client
Amy what makes you think he is ready?Amy – 19:42
His fluency has improved a lot, he seems a lot more confident and accepting of his stammer, we have put together a really good toolbox of strategies for him to use and he reports that these are working for him in everyday life and I am struggling with what to do with him in sessions now as we have covered everything in a lot of detailRachel Coulter – 19:43
Got to go here guys, sorry. Thanks for the adviceAmy – 19:44
he is worried that his dysfluency increases when he hasn’t had therapy in a few weeks so we definitely have a bit more work to do on maintenanceLisa Franklin – 19:44
bye rachelRosie – 19:44
Bye Rachel 🙂Amy – 19:44
Bye Rachel!Victoria – 19:45
bye rachelRosie – 19:45
Maybe spread the sessions out more, Amy? e.g. if you go weekly suggest you book another session in a month to see how he gets on?Rachel Coulter left the conversation – 19:45
Amy – 19:46
yea I have agreed with him that we will continue with 2 more weekly sessions and then reduce it to monthly but that he can still contact me by email in between this if there is anything he needs help withLisa Franklin – 19:47
you just need to manage the relapseAmy – 19:47
I’m hoping that if he has a month without therapy and everything goes well he will be more confident that he can use his strategies successfully without regular therapyLisa Franklin – 19:47
so I do reduce to monthly then 3 months. then if no issues after 2 x 3 months apts we discharge
some clients feel if the therapists goes the fluency might so they need that reassuranceAmy – 19:49
ok great, I will discuss doing monthly and then 3 monthly with him this week. I think you are right that he just needs a bit more reassurance that he isn’t just being abandoned once regular therapy finished
*finishesLisa Franklin – 19:50
yes absolutely
and he is quite young yes?Amy – 19:51
yea only 18
also just about to start his a levels and going to uni in sept so I think I need to make sure he understands that I can offer him more support during that time as it will be challenging for him when meeting new people etcLisa Franklin – 19:52
yeh my young clients are also more reluctant on discharge
yes absolutely as anxious times may cause the relapse so important he is prepared for relapse and those bad days
Okay anyone not checked in?Amy – 19:54
Yes definitely, thanks for the advice 🙂Rachel – 19:55
1) Tired – work life balance – need to be more organised! 2) Completed a Talk Tools assessment. 3) It’s my birthday on Sunday : )Lisa Franklin – 19:57
Okay talk toolsRachel – 19:58
4) advice re: talk Tools. What do you do if they can’t maintain enough grip on the jaw graders to keep it in their mouth at all. Obviously they have a weak jaw but where do you go from there??
Not sure if teeth alignment may also be affecting the ability to grip?Lisa Franklin – 19:59
chewy tubes
I had this with a client and she would chew it and not grip and it falls outRachel – 20:01
She’s 11-years though, so understands what I’m asking her to do?Lisa Franklin – 20:03
yes so its encouraging the grip, perhaps visual or tactile support
to try to get it held for 1 secRachel – 20:03
okLisa Franklin – 20:03
usually once they can hold it you can then build up the strength
does she use the chewy tubesRachel – 20:04
I don’t think so – I’ll try one
Actually she said she had a Talk Tools assessment when she was a lot younger – can I get this info from her case manager?Lisa Franklin – 20:05
if they have the report yes
the chewy tubes are usaully the first stepRachel – 20:06
ok, will go for that – thanksLisa Franklin – 20:07
encourages the biting down
Who else’s 4) has not been covered?Victoria – 20:08
mine hasn’tLisa Franklin – 20:09
Yes B
His speech sounds have developed really nicely since I saw him last. He is still mostly talking in nouns, he uses some verbs and adjectives (not words like big, fast etc). He’s recently started learning prepositions at school. Utterances are still quite short, but sometimes he can speak at 3kw level.
MUm reports he has been getting really anxious, he’s very routine based and at school when there’s a lot going on (possible sensory overload) he becomes very upset and withdrawn, school remove him from the situation. Can you suggest any anxiety strategies? I wondered also if simple social stories may be useful and a sensory programme?
Over summer we are going to do weekly sessions, I wondered if clinic may be more suitable as there are still a lot of distractions at home but I wonder how he will cope with a new unfamiliar setting.
Hmm note sure clinic is approp for him VictoriaVictoria – 20:11
me neither but I thought it might be a better use of time for me over summerLisa Franklin – 20:11
More so with risk ax
with his mobility he would be high risk if a fire were to happen and we need to risk assess for vulnerable clientsVictoria – 20:12
also i still don’t feel the parents are actually doing the therapyLisa Franklin – 20:12
Does he use visual timetable/now next/timer etc?Victoria – 20:13
he won’t sit to a timer, i’ve been using now and next boards with him
mum says they have a visual timetable at school
they’ve started doing ??do disco? at school and he becomes anxious and upset and wont do it
school haven’t been using any anxiety strategies they just remove him from the situation
he still hasn’t got the concept of big/little, i do wonder if the parents are pracitising this thoughLisa Franklin – 20:15
okay I learnt recently that we should teach concepts as big/not big
teach one concept at a time rather than comparativeVictoria – 20:15
oooohRachel – 20:16
Is there any OT input? Or could there be?
For the sensory issues?Victoria – 20:16
he has seen one in the past
he also briefly saw a salt in school but i don’t think they chose to do anythingLisa Franklin – 20:17
Leeds I must say are awful for SIVictoria – 20:17
his attention and listening is still very difficult, if he doesn’t want to do something he wont and he’s always off into his toybox to get things
part of me wonders whether a small group would be useful for him, to help him learn such skills such as turn taking from othersLisa Franklin – 20:18
I wonder if you could use an egg timer instead of sand timerRachel – 20:18
Do you give him reward time?Victoria – 20:18
yes, i let him choose from 2 activities at the endRosie – 20:19
I’m afraid I have to go, guys! not covered by number 4 yet but it’s not a huge issue so I will probs send out an email next week when I have some more info 🙂Lisa Franklin – 20:19
Or you could use the task breakdown countersVictoria – 20:19
bye Rosie 🙂Rachel – 20:19
Bye RosieVictoria – 20:19
so have a picture of each task and mark with a counterLisa Franklin – 20:19
Bye Rosie, please email me for advice on 4)Rosie – 20:19
Will do 🙂Amy – 20:20
Bye Rosie!Rosie left the conversation – 20:20
Lisa Franklin – 20:20
Or if you are doing a task – as simple as ‘good listening’ he needs to do 5 good listening to finishVictoria – 20:21
so do you think right back to basics, eg good listening, good sitting
and 5 counters then rewardLisa Franklin – 20:21
http://www.pecs-unitedkingdom.com/shop/product_info.php/cPath/23/products_id/77Victoria – 20:22
will favourite this 🙂Lisa Franklin – 20:22
you need to get the attention, and cause and effect e.g. ready stead – (wait for joint attention and good sitting) – go
they come in 1 – 5 counter boardsVictoria – 20:23
do we have any of these?Lisa Franklin – 20:23
so you can do really low level like you ask one request and he gets the reward/symbol
We do indeed but I have quite a lot of them but we need another pack so happy for you to order and claim back
the large ones are better than the small ones
This sometimes works better than ‘timers’ as there is a clear end and count downVictoria – 20:24
ok will do
Lisa will you be sending us a copy of all of this so I can read back through it?Lisa Franklin – 20:26
yes will doVictoria – 20:26
greatLisa Franklin – 20:28
okay all well if there is AOB?Victoria – 20:29
is colourful semantics training going ahead?
or shape codingLisa Franklin – 20:33
This is still to be confirmed 🙁Victoria – 20:34
is there any chance you could do a session on shape coding?Lisa Franklin – 20:36
We have the resources Victoria but I have not completed the courseVictoria – 20:36
ahh
well i’m going to have to head off now 🙂
thanks for the advice lisa, night all xxLisa Franklin – 20:40
